甲状旁腺切除术时机对移植肾功能影响的Meta分析

Abstract
目的：探讨甲状旁腺切除术（PTx）时机对肾移植患者移植物功能及预后的影响。方法：系统检索PubMed、Web of Science、知网等数据库（建库至2025年2月），纳入比较移植前（Pre-RTx）与移植后（Post-RTx）PTx的队列研究，采用RevMan 5.4行Meta分析。结果：最终纳入6项研究（1032例患者），Pre-RTx组412例，Post-RTx组620例。Meta分析显示，Pre-RTx组可降低持续性低钙血症风险（OR=2.89，95%CI：1.67～4.99，P<0.001），减少术后12-36个月eGFR恶化风险（P<0.05）；Post-RTx组术后1个月内20%患者eGFR下降>25%，且12-24个月为移植后PTx的最佳窗口期。两组手术成功率无显著差异（OR=0.92，P=0.64）。结论：肾移植前PTx可能更有利于保护长期移植物功能，尤其适用于重度继发性甲旁亢（SHPT）或合并骨病患者；轻中度SHPT患者可考虑移植后12-24个月行PTx。
