膀胱小细胞神经内分泌癌伴尿路上皮癌1例报告并文献复习

Abstract
目的：探讨原发性膀胱小细胞神经内分泌癌伴高级别尿路上皮癌的临床特点及病理特征、治疗及预后，以提高诊疗水平及患者生活质量。方法：回顾性分析1例女性原发性膀胱小细胞神经内分泌癌伴少量尿路上皮原位癌的诊治情况及病理特点，并复习相关文献，综合分析膀胱小细胞神经内分泌癌伴尿路上皮癌的流行病学特点，病理和免疫组化特征、临床表现、诊治经过及预后。结果：患者行根治性膀胱切除术+尿流改道术，术后病理提示：（膀胱）低分化癌，结合免疫组化检查结果考虑为小细胞神经内分泌癌，伴少量高级别尿路上皮癌，肿瘤浸润至深肌层外脂肪组织，侵犯脉管，未见明显神经侵犯，送检（双侧输尿管断端）阴性，送检（左闭孔淋巴结0/11）及（右闭孔淋巴结0/15）均未见肿瘤组织转移。结论：膀胱小细胞神经内分泌癌是一种罕见的高度恶性泌尿系肿瘤，具有独特的病理特点，转移较早且预后较差，目前治疗方法主要为膀胱根治性切除术联合术后化疗。
