微创与开腹胰十二指肠切除术治疗远端胆管癌疗效的 Meta分析

Abstract
目的：评估微创胰十二指肠切除术(MIPD)与开腹胰十二指肠切除术(OPD)在远端胆管癌中的疗效。方法：我们使用与胆管癌和胰十二指肠切除术相关的关键词,在PubMed、中国知网和万方数据库中进行检索,直到2024年12月。两位研究者独立筛选文献,提取数据,评估质量,并使用Review Manager 5.3进行统计分析。结果：共纳入七项研究,涉及1726例(714例MIPD,1012例OPD)。Meta分析显示,MIPD的术中出血量较少[MD=-161.71,P=0.0002],住院时间较短[MD=-2.36,P=0.0005],但手术时间较长[MD=57.46,P=0.0006]。术后并发症如胰瘘[OR=1.07,P=0.67]和胆漏[OR=0.92,P=0.72]未显示显著差异。结论：MIPD在远端胆管癌中是安全且可行的,尽管手术时间较长,但相较于OPD具有优势。
